I accept that the choice for a tattoo is completely my own choice.

I declare not to be under the influence of alcohol or drugs while getting a tattoo.

I understand that a tattoo makes a lasting change to my body. I am also aware that this is handmade by the artist and there is therefore always a chance of small imperfections.

I hereby declare that I have no mental or physical problems that could affect my health as a result of this tattoo.

I release the shop, owner and staff from any claim, expense and responsibility in the event of disagreement.
